Under such rules and regulations as are adopted by the Board, each member or someone
on his or her behalf shall file with the Board, in such form as the Board may prescribe,
a statement of the facts pertaining to his or her status as a member, which shall
include a statement of all service as an employee, and such other information as the
Board may require. Until such statement is filed, no member or his or her beneficiary
shall be eligible to receive any benefits under this Article. (20-81-3; 4-16-3.)
